Contrary to common perception, formulas such as Devine (1974) were not invented for aesthetic body composition evaluation. They were developed by clinical pharmacologists to standardize drug dosages (e.g., aminoglycoside antibiotics like gentamicin, theophylline, and chemotherapeutic agents) and calculate creatinine clearance. Because hydrophilic medications partition primarily into lean intracellular and extracellular fluid rather than adipose tissue, calculating dosage based on total body weight in obese patients risks severe drug nephrotoxicity and ototoxicity.
2. Formulaic Differences & Coefficients
Each major formula calculates ideal mass starting from a 5-foot (60-inch) baseline:
- Devine (1974): Men = 50.0 kg + 2.3 kg per inch over 5 ft; Women = 45.5 kg + 2.3 kg per inch over 5 ft. (Remains the benchmark cited by FDA labeling and hospital clinical guidelines).
- Robinson (1983): Men = 52.0 kg + 1.9 kg per inch; Women = 49.0 kg + 1.7 kg per inch. (Derived from Metropolitan Life Insurance data).
